MCPL Online Library Card Registration

Complete this form to request a library card.

You won't have access to library services until you receive your card. If you need immediate access, apply for a card in person at one of our libraries, bringing proof of address and photo ID.

If you apply online and choose to have your card mailed to you, you must bring proof of address and a photo ID to a branch within 30 days or the card will expire.

I wish to pick up my card at my home library branch: Yes - You may pick up your card in five business days. No - You should receive your card by mail within 7 days. Within a month after receiving it, you must go to a library branch to authenticate your card. If you do not, it will expire.
By submitting this form I accept responsibility for all materials borrowed and/or all costs incurred. I will report theft or loss of the card or change of address to the library immediately. I understand that this card can be revoked at any time.
