| Provide In-Home Aide Services/personal care service and/or chore service to frail elderly and adults with disabilities to help them remain in their own homes. An assessment is required to determine eligibility for the program. Eligible clients must agree to pay any fee required on a sliding fee scale. In addition, emergency temporary back up coverage for personal care services is available for residents who use the Medical Assistance Personal Care Program. To be eligible YOU MUST:
Be a Maryland resident AND
Be between the ages of 18 - 64
Be determined and certified by your physician to have a severe disability that keeps you from performing essential activities of daily living, self-care, and mobility; AND
Not be receiving duplicative attendant care services on the SAME Day; i.e., you could receive service from one program Monday-Friday and the other on the weekend
Have a total gross income(taxable and non-taxable) of less than $53,522 per year
AND
You must be employed: OR
You must be actively seeking employment; OR
You must be enrolled in an institution of post secondary or higher education;OR
You must be a nursing facility resident who would be able to reside in the community if attendant care is provided; OR
You must be at risk of nursing facility placement if you do not receive attendant care services in the community.
Maximum reimbursement amount is $308.75 bi-weekly based on an income from $0 - $20,000 per year, and the reimbursement goes down from there.
It is recommended to have the State's Medical Assistance Personal Care (MAPC) services or the County's In-Home Aide (IHAS) services in place prior to applying to the State's Attendant Care Progam. |

| The Maryland Department of Health and Mental Hygiene administers the Maryland Community Choice or Living at Home Waiver that provides services for those eligible individuals that meet a nursing home level of care and are between the ages of 21 and 65. This waiver is designed to provide consumer-directed personal assistance services for adults with physical disabilities in their own home. Waiver services include attendant care, administrative case management, family and/or consumer training, skilled nursing supervision, personal emergency response systems, environmental accessibility adaptations, and occupational and speech/language therapies. Persons receiving Medicaid and living for at least 30 days in a long term care facility are automatically eligible for the Living at Home Waiver. If the person receives both Medicaid and Medicare the wait is at least 60 days before being able to apply for waiver services. |

| Medicaid Waiver for Older Adults |

Persons eligible for this program are at least 50 years old, have a monthly income of no more than $1,911, assets that are no more than $2,000 to $2,500 and must meet medical eligibility requirements for a nursing home. Services may be provided in the home or in an assisted living facility. These services include personal care, respite care, home-delivered meals, environmental assessment family or consumer training, personal response system, etc. A list of individuals interested in receiving waiver services is being maintained.
To place a name on the Waiver Services Registry, call 1-866-417-3480 (Toll Free). Registrants will be notified as openings become available.
Waiver services are available immediately for eligible nursing home residents (wishing to leave the nursing home), whose stay has been paid by Long Term Care Medicaid for 30 days or more.

| The REM Program is a Maryland Medicaid Program that is part of the Maryland's managed care system known as Health Choice. It is intended to provide specialized services to people with significant health care needs who are eligible for Maryland Medical Assistance. To be eligible the person must have a diagnosis that is included in the program's criteria. The individual's special needs must be considered to be outside the scope of services provided through Medicaid managed care organizations. This is not a waiver program. |
