Filing Consumer Complaints


If you would like to file a complaint with the Office of Consumer Protection, please use the links below to download a complaint form.  To view a representative list of the types of complaints that OCP accepts, please click here

Please mail the completed form, with copies of all invoices, receipts or correspondence to:

Upon receiving a written complaint and necessary documentation, OCP investigators will contact the merchant and attempt to mediate a resolution. The consumer will be kept informed of the progress of the case and participate in negotiating a settlement. Cases are also reviewed for a pattern of deceptive business practices and OCP may take legal action to correct these practices if appropriate.
