- How to File a Complaint with MOSH and FAQ's
To report hazards at your worksite to MOSH you must be a current employee or an authorized representative of a current employee. Visit the MOSH website for more information or click here to download the MOSH Complaint Form in MS Word format. Complete the form and then fax or mail it to the address indicated on the form.
